Looks like it was the Oil pump drive pulley that exploded! Yikes, the pulley was in about 8 pieces, have a new OEM and new OEM Timing Belt on the way. There were metal shardes everywhere. Any idea on how this happened and how I can prevent this from happening again? Should I be replacing the oil pump as well? 

Who else has this happened to?

Yeah, they should be able to tell by looking.  The tensioner should be fixed in a tensioned position.  I suspect it may have been left bolted down fully retracted (in the belt R&R position), or not bolted down at all.

The shop manual gives good instructions on timing the cams and tensioning the belt properly.  Locktite wouldn't be needed or advised.

The motor could run for quite a while before the lack of proper tension causes the belt to jump...
