Mike,
What bothers me is the 12 volt output @ idle. If the alternator / regulator is performing properly, it should produce around 14.4 volts @ idle.
Even with a defective battery which will not pass a load test or hold a charge, the alternator will still produce over 14 volts in an attempt to charge the battery, unless the battery is completely dead.
Perhaps you have a wiring issue preventing the battery from recharging? Perhaps the regulator is failing once it gets hot? Clip your DMM leads to your battery terminals. Place the DMM where you can see it through your windshield and start your car. The voltage should drop to around 11 volts when the starter motor is engaged. As soon as the engine starts, the voltage should jump to over 14 volts. This simple test will indicate if your charging system is functioning correctly.
If a battery is not fully charged, it can fail a load test. Charge it and it can pass.
I still maintain what you describe indicates a charging issue, not a battery issue.
